What is High Fidelity Wraparound?

An intensive, individualized, team-based approach that helps young people and families with complex needs build stability in their homes, schools, and communities.

Family members, natural supports, schools, providers, and involved systems work together around one plan shaped by the family's strengths, culture, voice, and goals.

Our approach:

• Family voice and choice guide planning
• Strengths and culture shape solutions
• Support is coordinated across systems

What support can include:

• A dedicated team and individualized plan of care
• Team meetings, safety planning, and crisis support
• Peer support, family skill-building, and stronger protective factors
• Home- and community-based services that reduce transportation and scheduling barriers
• Connections to schools, community services, and clinical care, including psychiatry and medication management when appropriate
• Stronger natural supports and transition planning

3. Referral Sources and Partner Systems

Who can refer?

• Families and self-referrals
• Hospitals and behavioral health providers
• Schools and educational partners
• County and community agencies

Is Wraparound right for your family? This program may be a fit for youth who:

• Are ages 0-25 and have full-scope Medi-Cal coverage
• Experience emotional, behavioral, or mental health needs that affect daily life
• Are at risk of school failure, family disruption, out-of-home placement, mental health hospitalization, or juvenile justice involvement
• Need intensive coordination across schools, behavioral health providers, and community services
• Need a stronger support network because natural supports are limited or family stress is high

Eligibility is confirmed through the intake and assessment process. If you are unsure, contact us — our team can guide you through the process.

4. How to Make a Referral

A simple way to begin

Families may self-refer. Schools, hospitals, behavioral health providers, county partners, and community agencies may also submit referrals.

1. Discuss the referral — Confirm that the youth and/or caregiver agrees to be contacted.
2. Complete the referral form — Scan the QR code below or scroll to bottom of this page to complete the referral form online.
3. Submit the referral — Submit online, download and email the completed form to Wrap.OC@newalternatives.org, or call us at (714) 726-0050 to complete the referral by phone.
4. Our intake team follows up — We review preliminary eligibility and contact the family or referral source.

Services are voluntary. A referral begins the screening process and does not guarantee enrollment.
